PREQUALIFICATION: At least 5 days before the bid opening date prospective bidders must submit a notice of intent to submit a bid on bidder’s company letterhead with a name and e-mail address for the company contact person, a copy of the company’s current corporate registration with the Florida Department of State (SunBiz.org), and a current copy of State Contractor’s and Business License required by DBPR for the respective trade. PUBLIC ENTITY CRIME INFORMATION STATEMENT: Under section 287.017 Florida Statutes, a person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services and/or construction or repair of a public building or public work and may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity for a period of 36 months from the date of being placed on the convicted vendor list.
